do I register as a new or temporary patient?
Map of Practice Area
The practice map defines the area which
patients must live within to be eligible for registration at
the practice. We are unable to keep patients on our list who
move out of this practice area.

Registration Form
You will need to complete a registration
form to register at the surgery.
Initially we will ask you to complete a
health questionnaire as it is important that we have as much
information as possible on your past medical history. We
will, of course, be asking the appropriate authority to send
your medical records once they have been retrieved from your
previous General Practitioner, but this will take at least
2-6 weeks. You will be offered a new patient health check
with the practice nurse.
Temporary Patients
Anyone staying for up to 6 months can
register. If you will be staying more than 6 months, you
will need to register as a new patient. |
