Patients must live within the practice boundary to be eligible for registration at the practice. If you are unsure if your home address is within our boundary please contact us for confirmation. We are unable to keep patients on our list who move out of this practice area.

Registration

You will need to complete a registration form to register at the surgery.

If you are unable to register online, please telephone the surgery so we can confirm you are within our boundary. You will then be able to come into the Surgery to complete a paper registration form.

Initially we will ask you to complete a health questionnaire as it is important that we have as much information as possible on your past medical history. We will, of course, be asking the appropriate authority to send your medical records once they have been retrieved from your previous General Practitioner, but this will take at least 2-6 weeks. You may also be offered a new patient health check with the Practice Nurse.

If you have not received confirmation of your registration form being actioned by the practice within 14 days of posting it to the practice or handing in to Reception, please contact us. 

Allocated GP

From April 1st 2015 all patients have a named allocated GP. Your named GP will be either Dr Marquis or Dr Saggar and this will be recorded on your records. If you wish to change your named GP at any time please inform the Receptionist. You can still make an appointment with any of the GPs.

Online ‘Pre-Registration’ With The Practice

When you register you may be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us.

If you have not received confirmation of your registration form being actioned by the practice within 14 days of submitting it on line, please contact the practice on 01270 780210, thank you.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
